This portal is for an insurance company to use to respond to a Department of Insurance request for information about a consumer complaint.
To file a new complaint, visit the NAIC Consumer Information Source web site and select AZ from the map.
Consider saving all the documents that you will need to upload in a common location on your computer or network to make uploading easier.
Make sure to include all the documents requested in the Request For Information (RFI) notice you received, separated into the following categories:
Files must be in one of the following formats: Word (doc or docx), Excel (xls or xlsx), Adobe Acrobat (pdf),
or one of the following graphic formats: jpg, jpeg, gif, tiff, png, or bmp.
Do not combine files into a single ZIP file – this portal will allow you to upload multiple files at the same time.
